Enrollment Information

Annual Enrollment

Open enrollment is your annual opportunity to review, update, or enroll in employer-sponsored benefits for the upcoming plan year (September 1 – August 31), with all elections and changes required to be completed during this period unless a qualifying life event occurs.

New Hires

New employees have 31 days from their hire date to enroll in benefits, which take effect on the first day of the month following your hire date, and completion of the enrollment process is required to ensure timely coverage and avoid lapses in benefits.

Midyear Changes

The benefits you select will remain in effect for the plan year (September 1 – August 31), with changes allowed only if a qualifying life event occurs; in such cases, employees have 31 days to notify their employer and complete the necessary paperwork.

Continuation of Coverage

Upon retirement or termination, you may be eligible to continue certain insurance coverages through COBRA, Portability, or Conversion, allowing you to extend, transfer, or convert your benefits based on eligibility and plan provisions.

The information found within the benefit portal is designed to assist you in making important decisions regarding your benefits, provide you with important contact information for the benefit plans that are available to you.  Combined Benefits Group (CBG) is the Third Party Benefit Administrator for the District’s benefit program.

Combined Benefits Group

Medical

Major Medical is provided by TRS ActiveCare.  Primary and Primary Plus require a PCP from the Blue Cross Blue Shield statewide network.  ActiveCare HD does not require a PCP and provides access to the Blue Cross Blue Shield nationwide network of providers.

TRS ActiveCare

Dental

Sun Life offers a choice of three dental plans depending on your needs.  The enhanced option is your traditional dental plan that allows in-network benefits as well as out of network coverage.  The basic plan, offers an affordable alternative but reimburses non-network providers at the Maximum Allowable Cost (MAC).  When selecting the basic plan, it is important to utilize a network provider or understand that your coverage will reimburse non-network providers at approximately 50% of traditional plans.  Finally, employees have the option of the DHMO which features no deductibles and no maximums.  Under the DHMO, participants will be assigned a participating dentist office and must use this provider.  However, you may contact Sun Life to change your provider throughout the year.

With telehealth, you can receive treatment for minor illnesses without visiting a doctor’s office. Hospital Indemnity

As deductibles and co-insurance continue to rise, Hospital Indemnity provides cash directly to you when most needed.  Upon hospital admission, the plan will pay $1,500 to $3,000 plus a daily rate.  You can use the cash for any reason.  It is a great benefit for family planning

MASA is designed to prevent the balance billing that occurs when transported by ground or air ambulance.  Members have saved thousands of dollars by having this supplemental coverage when they receive a medical transport.   One low premium covers the entire family.

Disability

Disability Income Protection helps protect one of your most valuable assets—your ability to earn an income.

When choosing your coverage, you can select an elimination period, which determines how long you must wait before benefits begin. Available options include 7, 14, 30, or 90 days. In general, the longer the elimination period you choose, the lower your premium.

If you select an elimination period of 30 days or less and are admitted to the hospital, your benefits will begin on the first day of your hospital stay. This feature can be especially valuable when planning for the financial impact of starting or growing your family.

If disabled, benefits will continue to normal retirement age, so long as you continue to be unable to work and under a doctors care.   See policy for restrictions and exclusions.
